INSURANCE POLICY
We file all insurance claims as a courtesy for our patients. We are in-network for some groups on the following Insurance plans. We encourage you to call the member services number on the back of your insurance card to verify that Dr. Don Mabry or Dr. Caleb Mabry are in- network for your particular plan.
Blue Cross Blue Shield – including Medicare Advantage Plan
Delta Dental Premier Plan
Careington Network – This network includes SOME Aetna commercial & Aetna Medicare, Cigna DVH, DenteMax, GEHA, Guardian, MetLife PDP PLUS and FED VIP, Mutual of Omaha, Renaissance, UHC Optum Dental, UMR, United Healthcare Medicare and Commercial, United Concordia (care PPO only) Cigna and others. However, networks are forever changing participation so please call the member services number on the back of your I.D. card to check that your insurance processes thru Careington network and Dr. Don Mabry or Dr. Caleb Mabry are considered in network for your insurance.
Humana Gold Medicare plans — WE ARE OUT OF NETWORK, however, Humana offers several plan that include out of network benefits. You can check with your Humana representative to learn more about these plans.
